Is it possible to extend this module so I can configure if I want to show the 'new window' icon... like the old extlink module did.

That's probably outside of this module's tiny scope, and I'd recommend doing that on the theme layer. It can be done with nothing but CSS...check out the BlueTrip CSS framework for an example of that.
