When using pathauto for on multilingual sites to set the path of book pages pathauto only processes the book page that had the last saved parent correctly. The book page that had a previously saved parent gets treaded as a regular node (default settings are used). An example to illustrate:

  1. Create a node in language a and translate it to language b (parent node)
  2. Add a book page to the parent and translate it to language b
  3. The book page in language b gets the correct path, while the one in language a gets a path based on default settings
  4. Now edit the parent node in language a (just hitting save is enough)
  5. Add a book page to the parent and translate it to language b
  6. The book page in language a gets the correct path, while the one in language b gets a path based on default settings

I have little idea about the problem, but it would help to know which tokens you are using in your patterns for book pages and what settings you have for the different languages.
