Updated to use Forms API and changed _user_authenticated_id() for DRUPAL_AUTHENTICATED_RID

Tested on Apache 2.0.54 using mod_auth_sspi.

... tested NTLM on:
Windows XAMPP (PHP5, Apache2)
Wndows 2000 / (PHP4.3.1 / IIS5)
Windows 2003 / (PHP4.3.1 / IIS6)
- using Firefox 1.5 (Linux and XP) and IE 6
