Small fix for a custom module

I'm in need of someone to update the function that build the forms inside a custom module, its a jpgraph (graphs works fine)
but after a 4.7 to 5.x update the forms used to publish content and date selection for reports are breaking the module

looks like the errors are http://drupal.org/node/64279#drupal-get-form and http://drupal.org/node/64279#node-type-settings
but as I'm not a programmer so couldn't fix it myself
