On upgrade from 5.x-1.1 to 5.x-2.0, !site_name is not replaced with [site-name]

Upgraded a site from 5.x-1.1 to 5.x-2.0 tonight. Module works fine, but the frontpage was left proudly sporting a title of !site_name

Not quite as bad as "Untitled Page", but still looks a little naff :)

Would be very helpful if the upgrade process picked up the earlier pattern replacements and substituted the new format replacements for them.
