Double friends?

I've received a report that it's possible for people to have two of the same friend; presumably this happens when an acceptance link is clicked twice in a row, but I'll do some digging. If this is the case, it should check to see if a friend record already exists before adding it. Make sure this isn't also an issue for bookmarked users.
