A system that allows a small image to be attached to any content type and publishing options used to determine whether it is displayed in its title area on teasers or in the content's own page, or both should be part of the core system.
This functionality is needed for almost any journalistic site, yet the plugins to do it are not maintained and have problems because they aren't considered 'core' functionality.
I'm not saying it should auto generate a small image from a large one, just allow inclusion of the image in the title and allow it to be turned on/off in publishing options, or even in user profile configuration.
It's important that it control teaser and the main display behaviour individually - so that by turning off the display on the main page the article can include a large version - thus the small version would only appear in teaser titles. This is currently only achievable with a plugin, or by hacking php into your content titles that switches off the image when not in a teaser.
Putting this in the core code would also solve theming issues with the pictures not being bounded properly and thus being allowed to poke out of the title area, which is another problem that must be faced when hacking around this at the moment.
