Is it possible to overlay video content (locally stored files, no ffmpeg)?

Does anybody know how overlay jquery_media? What is it mediabox class and how it works?

Hi all,

I would like to know if someone has figure out how to insert a turnstile over a video, in a selected time,  with text and form in order to  collect the viewer´s information? Example, to know more click here, or input you e-maill to send you more info. Just like featured in Wistia.  

Any idea?
