I moved some terms to a new parent with default options. The terms now appear in taxonomy.module UI many times over again nested within duplicates of themselves. In Taxonomy Manager, they disappear completely! See screenshot below.

I'd like to add that it was easy to clean up. The terms weren't actually duplicated, just somehow set to have themselves as parents many many times.
