Add a Mailchimp Block

I wanted to include the newsletter subscription form in other parts of my website, but there was no block to do this, so I added a mailchimp subscription form block.

I've only submitted a few patches to the community, so I'm not sure I've done the patch correctly. Please check it out and let me know if it needs more work.
