I can not get the images to appear in the editor. Where do I place this file?

Try getting the 5.x-1.x-dev version of this module, which has the buttons included, and see if that works.

The "WhizzywigToolbar.png" should put on yourr website root, mean where index.php located.
I still find a way how to put this file on "sites/all/modules/whizzywig/" folder.
I will try to upload 6.x-1.0-ALPHA1 soon, and see how we can collaborate to make this works. You can help by testing this initial version.
