When you submit fields in the Subscribe block you are taken to crmngp/subscribe to fill in the rest of the fields, but the data you entered on the block's form are not retained.

Go to Administer -> Site Configuration -> NGP COO/CWP API Integration and check the Debug checkbox halfway down, then save and resubmit the block form. On the full form page, you'll get a set of expandable text areas at the top with the first one saying ContactSetICampaigns. Expand that (and the response fieldset within) and copy and paste the response here.
