Install/uninstall hooks simplified and converted to DBTNG. CHANGELOG removed.

- manual schema install/uninstall no longer needed
- CHANGELOG redundant with VCS history, especially with the Git migration
- update_N hooks renumbered to adapt for both D6->D7 and D5->D7 migration
