I'm not really sure at what point this started happening. I had upgraded to panels 3 and I believe I did have the problem at that time. Due to other issues I uninstalled 3 and reinstalled 2.

When I add a pane in the content tab and then click save the pane that I added is gone after save. the others are still there but the new one did not get added.

This was a problem with my upgrade process. I think I forgot to delete something while upgrading. It's fixed now
