Date Popup checkbox missing

I'm using Drupal 6.12 and the latest dev versions of Calendar and Date. I updated to the dev versions due to this same issue in the previous versions. Everything that has a checkbox except Date Locale (and Date PHP4) is enabled. In admin, I can't enable Date Popup, though, because there is no checkbox at all. That area is blank.
I know there is probably a simple answer to this, but I have not been able to find it, and I would appreciate it if someone would point me in the right direction.

Anybody??? Surely I'm not the only one to ever encounter this. Makes me think that I did something wrong or that there is something wrong with my installation of Drupal.

I ran into this same issue with 6.13. It turns out that using the Firefox add-on Stylish (with 'Ad blocking per Gozer' enabled) was messing up the form. I disabled Stylish, and the checkbox re-appeared. Hope this helps.
