Menu item "admin/settings/filters/customfilter/%" does not work with "gaps" in fids

Description
When the fids are not in sequenced order (can happend when you delete filters, for example "1" for Filter #1 and "3" for Filter #2), the menu item "admin/settings/filters/customfilter/%" may not work. When you click on the name in the "List Filters" tab only an empty form is shown.
I have attached a patch to solve this.

I was trying to understand why the module was not working, right before to check the issue queue; now I know why.
