Cannot assign a "from name" for emails reply-to field

Issue Summary

Hi All,

I have found a couple of bugs in the code. User cannot assign from name, as from email address used instead. Then reply-to field in email headers cannot be assigned properly. There is the patch in attachment.

The uploaded version is working fine on my site.

Please review and if you find it worth enough, let me know how to submit it to the release.
