Invalid from address

For some reason when I use this module, it does not like the 'from' addresses it's being fed no matter what they are. When I messed around with the code and set the from addresses to be something like 'example@example.com' instead of 'User

I also noticed that one of the menu items has an access argument 'access tellafriend block', but there's no permission by that name.

I created a patch with some fixes of these issues plus some removal of what looked like dead code. I'm not sure if anyone else is having this same issue, but this patch has been useful for me.
