Mirror Book Hierarchy

This patch attempts to allow automenu to assign menu entries that mirror a book hierarchy. It creates a new option on the content type workflow which will override the other automenu settings on that type.

This needs to be tested further. I'm not sure how it would interact in a very complex menu structure. However, I've tested it in a site where one of the books has also been assigned to primary links. The child pages of the book now keep the primary link active.

(There are also a couple of minor formatting changes to bring the module more inline with coding standards.)
