Themes are pretty darn flexible, though you have to remember that the HTML and CSS has to play nicely with all of the modules you install (now and later).
If you're attacking your first theme, I'd suggest starting with one that is close to what you're after from the repository on this site (http://drupal.org/project/Themes).

There are several different theme engines ( http://drupal.org/project/Theme%20engines ), PHPTemplate seeming to be the most obvious and widely supported.
