Do you reccomend using sub folder for cck images (using imagecache)? Would be nice for organization, but is there any drawback for doing so? When dealing with thousands of images, it's nice to easily perform batch operations...would sub folders make it easier or more difficult?

It is good to have sub-folders to manage your images. On what basis you create sub-folders is your choice. You can create folders for every imagecache preset.
BTW, imagecache images are created on the fly, even if you delete all images from your imagecache folders, these will be re-generated when visited.
