Maybe some issue with my installation, but I don't think so and I haven't got time to investigate. For now I'm just going to create individual CCK fields instead of using this module. I'm just posting this to let you know.

This is not a bug. You need to update your formatting options for your country(s) to include the phone and fax fields.
