I still can't access it and it is no longer listed on the access control page.

there are no special privileges required to access the filerequests' settings page, you just need to have "administer site configuration" rights.

Which I do, so this is perplexing. What other modules could possibly be interfering...

are you the first admin user (e.g. uid = 1) ?

No, I'm UID = 2 but I have administrator permissions.
So if I'm UID 1 it should work?

the way drupal works is that UID 1 always has all rights.
Now I know where to look in order to reproduce\find this error

Applies the fix in the wrong CVS branch. An updated package will be available in 24 hours

Great. Logging in as UID 1 I was able to access the settings. I had some immediate ideas on what you could change there so I made a feature request.
