pls look at the attachement
I suppose it happend after some menu modification (reordering, deleting, adding positions) - but I'm not 100% sure

all browsers.
clearing browser/drupal cache does nothing

I have the same problem, except that it occurred right out of the box. I thought that it have something to do with interference from nice menus and disabled that to no effect.
