The add node reference shows on the content screen for unsupported widgets, for example node reference from URL.
A quick check on widget type would remove it.
Could also possible implement a way for node reference widgets to declare how to use them, but that'd likely involve a lot more work and may be better left to those making the widgets. Though, node reference widgets (the complete 2) I know of tend to use the existing node references types, so perhaps it wouldn't be too ugly to do.
