How to use hierarchical select module?

Hi there.
I installed hierarchical select module (you can watch a demo here: http://wimleers.com/demo/hierarchical-select/taxonomy).

I've read the documents on project page, but still don't know how to create a block with a hierarchical select list, so user could use this awesome module.
If anybody knows, I will appreciate it.
