Option to include header and footer on node views is missing

Issue Summary

The docs for D5 say:
> # Check the box Include header in nodes? to include the header when viewing nodes with issues of the newsletter. The header is always included in e-mails with issues of the newsletter.

This is missing in beta 2.

So... does the D6 version include the headers always, or not?

I don't think so, anyways, the option does not exist. It might be a feature request, if you want to.

Well I think that's what I meant when I filed this -- we appear to have lost a feature!
Was it by design to drop it?

Yes, this feature is missing in the D6 version, and the header and footer is never displayed on the node view. The D5 feature should get back into the D6 version, or always display the header and footer.
