To replicate:

Create or navigate to a page with a custom alias.

Make a change and save the page.

Try to navigate to the page's custom alias. The page can not be found.

-- in the URL aliases under site building, the custom alias has been overwritten with an automatic alias

The custom alias must be re-set and re-saved in URL aliases every time the page is saved.

I've had the same if not similar problem with version 6.16.

Here's the sequence --
Create a new page, add content
Click the back button to navigate back to the "create content" menu
Click "page" to make a new page, add content

==== repeat this sequence several times ===

now click on "administer/URL alias"
the pages you've just created do not appear
They are registered in the db though, as the pages do appear under "Menus", and in manage content
