Issue Summary
I am playing with the Fivestar module, and each time I enable "Fivestar Extra module," I get the following outcome:
NOTE: I am using the Garland Theme.
1> enable Fivestar Extra 6.x-1.0-beta3 module
2> reply to a comment
3> type something in the body of the message
4> click on the "Preview" button
5> check the preview of the message
6> click "Save"
7> my replied message is not posted. It is missing.
At step 4 above, if I choose "Save" instead of "Preview," my replied message is posted to the comment.
I tried the same with the "Add new comment" with Fivestar Extra enabled. There is no problem. The comment is posted whether or not I choose "Preview" --> "Save" or "Save".
It seems that the missing message happens only when I "reply" to a comment with fivestar extra enabled and select "Preview" and click on "Save".
Lastly, with Fivestar extra disabled, the problem disappears. Everything works great.

I think this is indeed a bug the module causes. I am looking into it and will fix it as soon as I can.

Although I did see this happen last week, I am having trouble reproducing the problem, and so I can't fix it.
Can you cause the problem 100% of the time with Fivestar Extra enabled? If so, could you try disabling as many other modules as possible except for Fivestar Extra and see if the problem still occurs?
If it does, try changing the comment settings for that content type. See if some of the preview options make any difference.
As soon as I can reproduce this error, I will fix it.

I have following modules installed:
1> Fivestar Recommender 6.x-1.0-beta2
2> Recommender API 6.x-2.0-beta3
3> Fivestar 6.x-1.18
4> Fivestar Comments 6.x-1.18
5> Fivestar Extra 6.x-1.0-beta3
6> Voting API 6.x-2.3
It seems that when "4> Fivestar Comments 6.x-1.18" is enabled the problem happens. I did the following to enable it:
Administer -> Site Configuration -> FiveStar Comments Rating -> put check on "Enable Fivestar rating on comments"
Each time I disable "FiveStar Comments Rating" the problem disappears. It returns when I enabled it.
I hope you could duplicate the problem. I have tried this on Windows XP and Windows 7. I was able to duplicate the problem on both OSs. Once again, "Add new comment" is immune to this problem. It works in any conditions.

I believe this issue is fixed with the new release of the module. Fivestar Extra forms don't appear on the comment reply preview page now, but I don't think that really matters in comparison to fixing this bug.
