Make category list configurable

Issue Summary

It would be nice if the category list were configurable, so you didn't have to show Apparel if it was irrelevant to your site, for example.

This is made slightly more difficult because the categories are different for different locales.

Not only should categories be configurable, but we should leave out the ones (like mp3 downloads and kindall books) that aren't operable via the API.

There was a bug where "All" was added to the list and it was removed instead. This patch fixes it.
