Ask Question block fails in Panels

(Not sure if this is a Panels or Ask Question issue. I have cross-posted. Delete if posted wrongly here -- and forgive me, I hope..)

When I add the Ask Question block into a region in Panels 3x, I get an error when I click "Finish." It says "an error occurred while attempting to process /panels/ajax/ etc. You can see the full error message by clicking on the attachment, which contains two screen prints. The first demonstrates how two other FAQ blocks worked fine.

I did theme the Ask Questions node/add by using themekey module. Other than that, all permissions are granted up to and including anonymous. I have checked and rechecked.

I did find it interesting that the url being processed referred to /faq_ask_1 / form while when I look at the blocks page I see it listed at faq_ask/1 (see: /block/configure/faq_ask/1 )

POSTSCRIPT:

While I was not able to click Finish when attempting to insert the Ask block, what's very wierd is that after I closed the pane and was back on the content page, I clicked Save and ..... lo and behold...... now I can ONLY view the Ask Faq form when I click on either Preview or on Edit Content!

How is that for strange????

Do you get an error in the apache/php/watchdog error log? This does look to me like maybe that particular block is doing something strange and causing Panels to choke on it.

I'll have to look back in my logs and see what I can get for you -- after I figure out the other question you've been seeing me deal with ....the one about linking from a views block to a page with body text. With this issue, I ended up dealing with it outside of panels. Sorry I didn't return and close it. I think I should have rather than leaving it dangling as I did. Sorry about that.
