Using hook_theme to modify Drupal forms

Hello Jeff,

I was just reading this tutorial: http://adaptivethemes.com/using-hook-theme-to-modify-drupal-forms

and ran into 2 hiccups.

1. I was able to implement the changes to my comments form, however now I have duplicate content. Eg. I see two titles that say 'Comments' - see attached screenshot.

2. I wasn't able to get the code to theme the search form to work any at all. It gave a blank page :(. I had previously hidden the label using css, but was trying to follow your advice and do it the proper way.

I appreciate any feedback.
