Troubles with breadcrumb

I have problems using breadcrumb.
Even if I see an adress like "http://localhost/mutua/rimborsi/visite-mediche-ed-accertamenti-diagnostici" in the browser window, the breadcrumb always shows me adresses like "homeVisite-mediche-ed-accertamenti-diagnostici", without the parent directory.
Can you suggest me some operations that I can try to solve it?
