I'm about on the cusp of switching to a different anti-spam solution because I'm currently spending 90% of the time on my site deleting these types of comments, and it takes forever with the two-step confirmation. :( For now I've resorted to marking comments read-only after about a week on all of my posts, which is not a great solution at all.
My one final recourse would be a setting in the settings form somewhere for entering some kind of "always block" keywords. So if "juicy jewellery" or "wow gold" or whatever was found anywhere in the comment body, no matter if the captcha was passed or not, it would simply prevent the comment from being posted. Better still if this information could be fed "upstream" to Mollom and it could make adjustments accordingly.

I'm not sure how exactly this could be improved through the module. The best thing to do would be to always be reporting the spam content back to Mollom, which you can do through the admin/comments interface. I recommend using Fasttoggle to unpublish the spam comments when viewing each node, then you can use the mass-report and delete.
I do know that there have to be some alternative spam modules that provide custom/keyword filters. http://drupal.org/project/spam is the first one that comes up, but I can't find much more. This type of thing also seems like it would be more appropriate to not be included in the module.
Other than that, I'm not sure what else we could improve on. More ideas or suggestions?
