User password request form is set to "CAPTCHA only" but the CAPTCHA is not visible on the form.

Same problem but on user registration pages for me.

EDIT: Disabling and then re-enabling protection on that form fixed it for me. Also flushed Drupal's cache, so that might have something to do with it too.

If the CAPTCHA is not displaying, yes, it most likely is that problem. All you need to do is go to the Mollom settings page and just click 'Save configuration' without saving anything.

Thanks. I've actually updated to 6.14 and re-enabled Mollom on user password request form and the CAPTCHA is displaying as expected.
