I'd like to change what the user sees when they are asked for info.

Can you please be more specific. What exactly do you want the user to see?

Also, no further development will be done on the D5 version with the exception of bug fixes. Unless of course other people submit patches for it.

Rather than just show the file name and then have a field for the email address, I'd like to be able to add a description/instruction text, such as "Please provide the following information prior to downloading "doc name".

Also, it looks as if the document name takes on the styling for 'title'. If the style for the text had its own class then I could change the style in my css.

I believe that all that output is wrapped in a theme_ function that can be overridden in a template file. Let me know if that's not the case and I'll drop it in one. If you're referring to the 5.x version than I think that might not be the case.

I've added some extra text in the D6 dev version now, I agree that just a plain list of files doesn't really make too much sense... marking this as closed for now.
