Attributes Filter by Active/Default not working

Description
When applying filters to the Attributes table, setting the Active or Default fields to Yes/No has no effect on the results table.

Fixed. It seems that despite the fields being present on the form, the values were not being checked at all when the page is loaded.
Committed to 6 & 7 branches. Patch attached.
