Best way to make a consent form, once click "agree" cannot change

How to make a consent form that a certain role must fill and sign and once signed, cannot change?

This is like a profile page with many filling filelds plus a checkbox like the terms_of_use or legal.

Once "agree", the page become read-only with no more edit allowed.
