The module works for submitting a new comment. If the user the clicks Post new comment to post an additional comment, the comment form is displayed again, but this time it contains two textareas for the comment content. One slightly smaller below the main one. Any idea why?

I'm using Firefox 3.5.3 in OS X 10.6

This is more than a year old and I can't reproduce it. Please try with latest -dev and open a new issue if this is still applicable. It may be related to your theme, also try with Garland.

Just wanted to add that this bug happened to me too, in 7.x. I used a fresh install with Zen to see that it worked so, it had to be theme related or conflict with another module.

Anyway I copied the original comment.tlp and wrapper to my templates and everything is fine now. Just wanted to say so if it happen to someone else...
