All US card data sent to SagePay must have a field called BillingState sent (but only US cards ... wtf?) - current implementation does not do this, so American cards cannot succeed:

No bug, just the "Zone" field was not enabled in the address panes, so there was no way for someone to pick their State. Didn't affect any countries apart from the USA. =)
