Can anyone recommend a production site that has a good example of the Survey Module in action?

I have been playing with DP for about a week to see what it can do for a potential solution to a web site I am developing. I need to see what the Survey Module can do. After reading the Survey Mod info and installing it I still do not really have a good idea as to whether it is the solution I am looking for. I need a survey that will allow users to enter text, choose from prepopulated drop down list and select a rating) (from 1 to 5 for example.

I have not run into any sites that have the survey module running with a cool survey. If anyone can point me to one that would be "boffo".
