It would be nice to be able to produce unified diffs from drush. Not idea how easy/possible it is, but it would be awesome.

Problems are that the paths are wrong and that the diff is the wrong way around.

This issue is too vague. Drush integration works, will give you details about patched modules, and will diff projects for your leisure. Lovely. Will probably open a bunch of issues with bugs and features related to this one.

Does everyone feel like, sometimes, they are just talking to themselves?

Sometimes. Come on over to #446736: Have drush update be told to ignore a module/theme -- simple solution.
