I have captcha enabled, and for some strange reason it splits the Save and Preview button.

See the attached screenshots.

I've confirmed that this occurs with FF3 and IE8.

CAPTCHA does a best effort try to find a good spot for the challenge, but can't cope with all possible types of forms automatically. Please provide more info about the form.

Is it on a node form (custom content type) ?
Or a custom form?

Thats from core comment module form on a core story node.

(it does on my setup)

Do you have any other customizations on forms?
E.g. I see you are using the wp-comments module (but I doubt this one is the culprit).
It could also be a theming thing: what theme are you using? Can you try if the problem also exist with other themes? (I use garland on my dev/test sites for example.)
