I downloaded and activated the contact form module.

A created the categories as required, how can I create the forms and where to create them?

I am stuck and don't know what to do next.

There's a link to documentation right on the project page. Start there.

After a brief review, I would say you don't create the forms. As the docs say, this "extends" the site wide contact form, which you don't create or design. If you want to create your own forms, try the webform module.

And the best place to ask support questions about a particular module is in the issue queue for that module.
