User selectable countries and state/provinces

I've searched for hours and can't find any setting so to help with this...my site is in Canada but I want to let users create addresses in ecommerce to ship to Canada and the United States.
When creating an address the user's default is Canada and they are left with a blank field to manually type out their Province. They CAN select another country but when they pick 'United States' the State/Prov field changes to a drop down menu with the default title of 'Please choose' but the drop down menu is empty.
A few questions...:
- how do I get a list of States listed for the United States
- how do I reduce the list of Countries I want in the Country drop down menu
- how do I change the Prov field (when Canada is selected) to a drop down menu (instead of a blank text field) and have a list of valid provinces...
??
