I rest-ran Filefield Insert in the past and it worked for me, still does (I tested) and I had the HTML insert settings in the content type (as could be seen in the movie).
However, I tested the Insert module today on two computers (Windows/Xampp), both beta1 and beta2, and I get neither the HTML insert settings for the content type nor do I have an Insert capability when uploading pictures. Updating ImageField and Filefield to the new 6.x-3.2 version did not change anything.
As there is no documentation, I'm unsure if there's anything else I could do. It makes me a little nervous that the module works for others and not for me, but I'm running out of ideas. Help would be appreciated :)

quicksketchAh! I need to put on the project page, you MUST be running the latest version of CCK (2.5), since we're using new APIs that were introduced in that version.

zzrwood commentedI seem to be having the same problem - was working when it was Filefield Insert but is not working as Insert (I am running CCK 2.5).
I can upload image but don't get the ability to insert image at the cursor. I have attached a screenshot of the form field.
Hope to get it working again as it really is an excellent module.

quicksketchzzrwood, make sure that you've actually enabled Insert on the field you want to have it show up. It should have been automatically upgraded for you, but the beta1 version had some problems with the conversion. It'd be a good thing to double-check.

P.S. I think you need to add to the project page that the Insert settings have to be made per filefield in the content types. (Otherwise, you force all people to watch the movie :)

quicksketchYou need to enable it on the FileField or ImageField you want to have an Insert button. The insert button then works on any textarea (including the body).
