File attach disappears

I installed upload image mod on my D5 site and it's functioning oddly. I can attach 1 image just fine. If I try to upload a 2 image, everything dissappears. I have attached imgs to help better describe what happens. Both imgs are actually uploaded if you click submit, but it removes the "list" check mark from them. You can edit and check "list" and they will both appear, but why is it disappearing on me? Any ideas???
