Plazes error - admin/settings/plazes comes up empty

Description
The error I get in the watchdog (repeated dozens of times):
array_shift(): The argument should be an array in /home/[account-path]/html/includes/form.inc on line 469.
I'm unable to get the module to function with 4.7 clean install.
