I’m looking for consistency in the look of my site and would like to know how I could move the text fields just below the labels like what’s shown in the desired view image. Thanks.

You have to alter the label.printmail-label style in the print/css/printlinks.css file.

I think that deleting all of it will make it behave like you want.
