Recently added images not appearing in list of existing images to attach

In the attached images section on the node edit form, not all of my images are available in the existing images attach menu.
The most recently added images are just not listed, although they do appear in the list at /admin/content/node, the images are on the server at /sites/default/files/images (with all the thumbnails), and the nodes themselves seem to be fine.
Another odd thing is that the list of images is only semi-alphabetical - it has some of the images A-Z, followed by some of the other images A-Z.
Running cron has had no effect on the problem.
Also, I can no longer add new images directly via attached images. Nothing has changed in the system configuration since the last time I was able to add images.
I've tried adding the images as independent image nodes, but still they don't show up.
Is there a maximum number of existing images? Any other ideas? Thanks.

One other thing that might be relevant - when attaching a new image while editing the node, the thumbnail appears, but then after saving there is no attachment when the node is viewed, or edited again.

OK - I feel foolish now. It was because the images weren't being published by default. The existing image list only shows published images.
